Calculation sequence

Reference example
With 5,000 of eligible expenses, a 500 annual deductible and 80% reimbursement applied after the deductible, the uncapped payment is 3,600. A 10,000 remaining annual limit does not reduce it.
Boundary and error checks
Test zero-rate branches where available, minimum and maximum supported inputs, and a scenario in which the main result reverses. Reject blank, non-finite or out-of-range values rather than silently substituting them.
