    What this calculator answers

    Australian private-health costs can combine an income-tested rebate, Medicare Levy Surcharge and Lifetime Health Cover loading. Each has different rules and bases.

    Reproduce the published 2026–27 PHI tier arithmetic while keeping policy suitability and tax eligibility outside the model.

    Method and calculation order

    The 2026–27 income thresholds select a tier. Age and tier select the published rebate percentage; MLS applies only when the user marks no approved hospital cover. Simplified LHC uses 2% per starting age year over 30, capped at 70%.

    Worked reference example

    A single person aged 40 with 120,000 MLS income is Tier 1. The under-65 rebate is 16.079%; without approved cover, the modeled MLS is 1% of income.

    How to use the result

    Use ATO-defined income, confirm cover for every required family member and use insurer records for LHC base day, permitted days and prior loading. Compare policy coverage separately from tax.

    What the calculator cannot know

    Specific to 2026–27 thresholds and rebate percentages checked 2026-10-08. The LHC calculation is simplified and excludes permitted days, overseas and migration rules, ten-year removal and couples averaging. The ATO and insurer determine outcomes.

    No. The cover must satisfy current approved-hospital-cover rules for the relevant people.

    Is the 2% Medicare levy included?

    No. This tool models only the additional MLS.
