Recorded method

Verify each domain, warning-sign separation, conditional notice, and fixture arithmetic.

These records describe the published model and reference tests. A test-case count is not a certification of every possible input or an independent specialist review. Editorial policy

Known limitations

  • The 0 to 4 entry scale is descriptive and is not a licensed PROMIS score.
  • The tracker does not identify triggers, conditions, dehydration or treatment.
  • Urgent-warning fields are not exhaustive and cannot determine that staying home is safe.

What the Digestive Symptom Tracker records

Record digestive symptoms by domain and produce a printable summary without inventing an overall gut-health score.

It preserves pain, reflux, bloating, constipation, diarrhea and swallowing entries as distinct domains. The summary is not a validated PROMIS score and does not turn the entries into a universal gut-health grade.

How to use the Digestive Symptom Tracker

  1. Choose an entry date.
  2. Rate each symptom domain for that date.
  3. Mark urgent warning signs separately.
  4. Print or save the result and add context such as meals, medicines, or bowel frequency outside the tool.

Domain method and clinician-ready output

Each symptom uses the same descriptive 0 to 4 entry scale. The tool reports how many domains are present and the highest entered rating, while keeping urgent-warning items separate. Repeated entries can be printed or compared without implying a diagnosis.

Worked example and boundary check

Worked example

If abdominal pain is 2, bloating is 3 and diarrhea is 1 while other domains are 0, the result shows three domains present and a highest entered rating of 3. It does not average them into a gut-health grade.

Boundary check

A low count does not rule out a serious problem, and a high count does not identify a disease. Duration, onset, age, pregnancy, medicines and clinical history are outside this calculation.

How to use the record responsibly

Use patterns across dated entries to prepare a clearer history for a qualified professional. Treat new, severe, persistent or worsening symptoms according to current medical guidance.

  • The 0 to 4 entry scale is descriptive and is not a licensed PROMIS score.
  • The tracker does not identify triggers, conditions, dehydration or treatment.
  • Urgent-warning fields are not exhaustive and cannot determine that staying home is safe.

Frequently asked questions

Is the domain count a gut-health score?

No. It only counts how many entered domains are above zero.

Can it diagnose IBS, reflux disease or an intolerance?

No. Many conditions share digestive symptoms.

Should I wait for another entry when a warning sign is present?

No. Seek prompt medical advice, and use local emergency services for severe symptoms or immediate danger.

Can I compare entries?

Yes. Keep the scale and domains consistent, then compare the dated printed or saved records.
